Setting Up Water Service

All required sections must be completed in their entirety prior to submission. Upon completion, the full packet shall be presented to a Customer Service Clerk for formal review.All requested information is mandatory, and each individual page must bear the applicant’s signature before the packet will be accepted.
To establish utility service, customers are required to complete all forms contained within this packet.
The only form that is optional for filing is the ACH Authorization Form. While completion of this form is optional, enrollment in the ACH Draft program is strongly encouraged as a secure and efficient payment method.
ACH Authorization Form
Participation in the ACH Draft program is entirely optional and is not required to establish a water account with the City. Should you elect to enroll in this payment option at any time, please ensure that all required sections of the ACH Draft form are completed in full. The completed form must be returned to City Hall accompanied by either a voided check or a copy of your bank card displaying your routing and account number. This documentation is mandated for auditing and verification purposes.
